Diffuse Large B Cell Lymphoma is the most commum non-Hodgkin's lymphoma, and is also the mos commum in adults. It is classified as high or low grade, according to its agressiveness, and may have high mortality rates. Extranodal involvement is relatively common, and the main site is the gastrointestinal tract. In this case report, we present a female patient, 72 years old, with intestinal obstruction due to intussusception of the ileum to the cecum, secondary to tumor mass due to Diffuse Large B-Cell Lymphom. As treatment, a right ileocolectomy with retroperitoneal lymphadenectomy was performed. As immunohistochemical study, CD-20, MUM-1, BCL-6, BCL-2 and Ki-67 were positive and CD-3, c-MYC and CD-10 were negative. One month after the surgery, the patient underwent a PET/CT scan which did not show any active lymphoproliferative neoplastia. The most common sites of lymphomas in the gastrointestinal tract. Due to the large amunt of lymphatic tissue, the ileocecal region is the most commonly involved in primary intestinal lymphomas. As the disease is usually diagnosed late, the treatment performed is usually based on the combination of surgery and adjuvant chemotherapy.
