When the cecal appendix is present in the inguinal hernia sac, is called Amyand's Hernia, a rare type of hernia that has and incidence of 1% among all inguinal hernias. The inflamed appendix inside the hernail sac can cause signs and symptoms that simulate an incarcerated inguinal hernia, the diagnosis being made mainly during the operation. We report the case of and 82-year-old patient with painful, irreducible inguinal hernia and associated inflammatory signs. During the surgical procedure, the cecal appendix, bladder and small bowel loops were identified inside the hernial sac. Appendectomy and inguinal hernioplastic were performed on the right with marlex mesh apposition.
