Tuberculosis (TB) is a frequent bacterial infection in underdeveloped and developing countries. It is com-monly located in the lungs and lymph nodes. Genital organ involvement is rare and usually secondary. In this case report, we describe a case of diagnosed tuberculous salpingitis only diagnosed intraoperatively associated with a complex adnexal mass. Symptomatic genital tuberculosis presents with infertility, men-strual irregularities, abnormal transvaginal bleeding and abdominal pain. The most common manifestation of genital TB is tuberculous salpingitis through hematogenous dissemination. The diagnosis of tuberculosis is confirmed by histopathological examinations or bacteriology. The clinical condition of genital TB is varia-ble and able to mimic other pathologies, requiring strong suspicion for its diagnosis and intervention.
