Spigelian hernia is a rare abdominal wall defect occurring through the Spigelian aponeurosis. Due to its interstitial nature, clinical diagnosis is often complex. This paper reports the case of a 58-year-old male patient with a complaint of pain in the right iliac fossa, diagnosed via computed tomography and submitted to surgical correction by videolaparoscopy. The procedure was uneventful, with early hospital discharge. The minimally invasive approach proved effective for definitive diagnosis and anatomical treatment.
