O uso precoce de octreotide no tratamento de fístula quilosa pós-gastrectomia total oncológica: relato de caso

Resumo

A fístula quilosa é uma rara complicação pós-operatória de gastrectomia total com linfadenectomia D1/D2. Relata-se uma mulher de 50 anos submetida à gastrectomia total oncológica com linfadenectomia D2 e reconstrução em Y de Roux. No sétimo dia pós-operatório, observou-se no dreno de Blake secreção leitosa sugestiva de quilo, confirmada laboratorialmente. Instituiu-se terapia precoce com octreotida por três dias, com rápida redução do débito e conversão para drenagem serosa. A paciente recebeu alta no 11º dia, sem necessidade de reintervenção. Trata-se de complicação incomum e associada à dissecção linfática extensa no câncer gástrico, inexistindo protocolos terapêuticos padronizados e persistindo desafio no manejo. O tratamento conservador inclui jejum e nutrição parenteral total, com ou sem farmacoterapia. A octreotida, análogo da somatostatina, associa-se à redução do débito quiloso e do tempo de internação. O reconhecimento precoce e o manejo multidisciplinar são fundamentais, sendo a terapia farmacológica precoce potencial adjuvante no controle das complicações.
